What services does Pure Heart Community Care offer?

We provide NDIS support including school holiday programs, respite, The Hub (a welcoming community space), SIL (Supported Independent Living), and one-on-one community participation. We also offer in-home supports and hub-based activities for participants aged 7–16 and 18+.

Who can access your programs?

Many of our programs are designed for NDIS participants aged 7–16, including school holiday programs and respite for children and young people under 16. We also support adults 18+ with in-home supports, hub activities, SIL, and community participation—depending on your goals and NDIS plan.

How do I get started?

Contact us for a free initial consultation. We'll listen to your goals, explain how our services might fit, and help you understand next steps. Places can be limited, so we encourage families to enquire early.

What NDIS funding categories do you support?

We support NDIS Core Supports, Social & Community Participation, and Capacity Building funding, depending on your plan and the service you need. We can talk through your plan in your consultation.

Where are you based?

We're based in Werribee, Victoria, and support participants in Werribee and surrounding areas. Contact us to confirm we can support your location.

What is respite support?

Respite gives families a short break while their child or young person enjoys a safe, fun environment with trained staff. Activities are engaging and personalised—so you can recharge knowing care is in trusted hands.

What are your school holiday programs like?

Our school holiday programs are safe, inclusive, and fun for children aged 7–16. We focus on friendship, confidence, and trying new activities at a comfortable pace—with small groups, trained staff, and personalised care. One-day trips and transportation may be available (conditions apply).

What is The Hub?

The Hub is our welcoming community space where participants can connect with others, join activities, learn new skills, and feel a real sense of belonging. It's part of our broader person-centred approach to support.
