Our Story

At Pure Heart Community Care, we provide fun, safe and inclusive school holiday programs for children and young people aged 7-16 years under NDIS, as well as in-home supports and hub-based activities for participants 7-16 and 18+.

We tailor our support plans to meet the unique needs and goals of each individual, ensuring they receive the best possible care. Our respite care provides a safe, supportive and fun environment for children and young people under 16, giving families a short break while their child enjoys engaging activities, social opportunities and personalised support from trained staff.

Pure Heart Community Care is based in Werribee and trades under Eminent Care Services. We support NDIS Core Supports, Social & Community Participation, and Capacity Building funding, and encourage families to book early as places are limited.

With more than 15 years in disability support, Ta began on the front line as a support worker and grew through leadership into management and now serves as Director. That journey shapes how Pure Heart Community Care operates: grounded in real experience, focused on dignity, and committed to care that listens first. Ta builds lasting relationships with participants, families, and the wider community to deliver responsive, person-centred support you can trust.
